  7. I have no idea if this is like what is happening to you, but when I first went on plaquenil I got some weird pigment changes on my forehead some darkening that has ended up being permanent. My rheumy said that plaquenil can make you even more sun sensitive, so it looks like some sun exposure along with the plaq caused my pigment changes.

Plaquenil Hydroxychloroquine is used to treat and prevent malaria infection, and to reduce symptoms and progression of autoimmune diseases such as lupus & others. The risk of hydroxychloroquine-related retinopathy associated with skin hyperpigmentation is unknown.3 The risk of eye toxicity increases sharply toward 1% after five to seven years of hydroxychloroquine use, or a cumulative dose of 1000 g.4 About 25% of patients receiving chloroquine or hydroxychloroquine for several years develop bluish-grey pigmentation on face, neck and sometimes lower legs and forearms Continuous long-term use may lead to blue-black patches, especially in sun-exposed areas

My Take on New Ocular Screening Guidelines for Plaquenil. I am lucky as a rheumatologist in practice for more than 30 years to have only had one malpractice lawsuit. But it was for alleged Plaquenil related eye toxicity, and believe me, attornies will grab any guideline they can as "Standard-of-Care" if it suits their case. All to say, we treat patients, not guidelines, but we all need to aware of these.
